Quick answer

The rail pressure sensor is producing a signal in the right range, but one the engine control unit does not believe — usually because it disagrees with the pressure the unit is actually commanding. It is the code you get when a sensor is going lazy rather than dead.

How the car works this out

With the engine off and the system depressurised the sensor should read close to zero, and with the engine running the reading should follow the commanded pressure closely as load changes. P0191 is set when the signal is electrically valid but fails one of those checks: an offset at rest, a lag behind the commanded value, noise on the signal, or a persistent gap between the commanded and actual pressure that the control unit's corrections cannot close. That last case is important, because it can equally mean the sensor is lying or the pump genuinely cannot deliver.

What to do about it

  1. 1Ask for the commanded and actual rail pressure to be logged together under load. That single graph decides whether you are buying a sensor or investigating the fuel system, and it saves the most money of anything on this page.
  2. 2Check the rest-state reading with the engine off. A sensor that does not sit near zero at rest has proved itself.
  3. 3Change the fuel filter if it is anywhere near due. It is cheap, it is a known cause, and it needs doing anyway.
  4. 4Have the injector back-leak measured before agreeing to a high-pressure pump, particularly on a high-mileage common-rail diesel. Worn injectors and a worn pump present almost identically and the back-leak test tells them apart.
  5. 5Be wary of a pump that has failed by shedding metal. If it has, the debris travels through the rail and injectors, and a pump-only repair will fail again.

The car will usually get you there, but the limp mode is unpredictable and tends to arrive under load, which means on a hill or during an overtake. There is a second reason not to persist: if the underlying cause is a failing high-pressure pump rather than the sensor, continuing to drive risks sending metal debris through the entire fuel system, which turns one repair into a very large one.

Will it fail an MOT?

The lamp is a major defect regardless of the code behind it, one of 371,859 recorded failures. This code carries more emissions risk than the pure circuit codes, because fuel pressure directly determines how well the fuel atomises. Poor atomisation on a diesel means soot, and diesel smoke opacity limits are measured during the test; on a direct-injection petrol it shows up in the hydrocarbon reading.

How do I know if P0191 is the sensor or the fuel pump?
Have a garage log the commanded rail pressure and the actual rail pressure side by side while the car is driven under load. If the actual pressure tracks the command closely and the code still sets, the sensor's calibration is drifting. If a real gap opens up under load, the fuel system cannot deliver what is being asked of it, and the investigation moves to the filter, air ingress, injector back-leak and the high-pressure pump. Checking the resting reading with the engine off is a useful second test: it should be close to zero.
Can a blocked fuel filter cause P0191?
Yes, and it is one of the first things to rule out because it is cheap and often overdue anyway. A restricted filter starves the high-pressure pump, so the pump cannot build the pressure the control unit is asking for and the gap between commanded and actual pressure widens under load. On older diesels, a filter housing seal drawing air produces the same symptom and is just as common.

What is an injector back-leak test?
It is a measurement of how much fuel each injector returns to the tank instead of putting into the cylinder. The garage connects a set of graduated tubes to the injector return unions and runs or cranks the engine, then compares the volumes. A worn injector leaks far more than a healthy one, and because that leak bleeds rail pressure away, it produces exactly the commanded-versus-actual gap that sets P0191 and P0087. It is the test that stops a good high-pressure pump being replaced unnecessarily.
